Patient Profile
CrCl Calculation Method
Stable Formula
Use C-G when: dosing most antibiotics — drug labels & PK/PD trials were built on this formula. Default choice for routine clinical dosing. Does not require height.
Use CKD-EPI → mL/min when: patient has non-standard body size (obesity, amputation, extremes of height) or a more precise GFR estimate is needed. De-indexed to absolute mL/min via Mosteller BSA — requires height. Endorsed by FDA 2024 & NKF/KDIGO 2024 for drug dosing.
Kinetic Formula
SCr1 must be a stable pre-AKI baseline — outpatient or admission value before deterioration.
No stable baseline required — both readings can be from within the AKI episode.
Hepatic Function
Body Status
Note: Enter the patient's current (post-amputation) weight. IBW will be adjusted automatically.
Standard Function: Bypassing renal calculation. Dosing reflects non-impaired kidney function (>90 mL/min).
